Thermage delivers 30–60kJ into the dermis with AccuREP control. Who it suits, who it doesn't, and why the doctor's design decides the result.

The desire for anti-ageing has been strong in every era.
Functional cosmetics pour onto the market from tens of thousands of won to several million, and folk remedies such as sparkling-water cleansing or natural masks circulate online.
But what I feel meeting patients day to day is that care focused on the epidermis alone rarely achieves the turning-back-the-clock effect people truly want.
The fundamental cause of ageing lies not in the visible outer layer but in the dermis beneath it.
As collagen and elastin in the dermis change and decline, the skin loses support and begins to sag.
One of the most powerful answers modern medicine offers for change at that depth is Thermage.
In this column I want to look closely at why Thermage holds such a distinctive position, and why it cannot be the same magic wand for everyone.
The core principle of Thermage is powerful radiofrequency (RF) energy.
Without damaging the surface, RF energy is delivered deep into the dermis, where it generates heat.
That heat immediately contracts loosened collagen fibres while stimulating the fibroblasts that produce new collagen.
The numbers make the scale clearer.
A standard Thermage FLX treatment (600 shots) delivers total energy of roughly 30–60kJ or more into the skin.
Among non-surgical lifting devices available today, Thermage is almost the only one that can pour that much energy into the dermis in a single session while controlling the risk of burns.
It is best understood as a treatment that remodels the skin from within, laying the foundations again.
However good the result, a medical device has no value without assured safety.
Thermage has been upgraded steadily for around 20 years, from the first-generation model in 2003 to today's fourth-generation Thermage FLX.
The key technology to note in the FLX is the AccuREP automatic adjustment algorithm.
Skin resistance differs by area — the cheeks, jaw and forehead differ in thickness and water content.
AccuREP measures the resistance of each area in real time with every shot and optimises the energy.
This minimises the risk of burns while safely delivering the maximum energy that area can tolerate.
It resolves the central difficulty of anti-ageing — safely, yet as powerfully as possible — through engineering.
Here we must ask an important question.
Can Thermage give everyone the highest satisfaction? My answer is no.
Thermage is specialised in tightening.
It is excellent at firming slack skin and smoothing texture.
Thermage's own global guidance recommends it for those at the stage where sagging is beginning, or with mild to moderate sagging.
If ageing has progressed considerably, with severely descended fat or deep folds, Thermage alone will not produce the dramatic change you may expect.
For those patients, thread lifting to draw the tissue up directly, or volume treatments such as Juvelook Volume or Sculptra, should come first.
Or an approach combining it with Ulthera, which targets the SMAS layer, may be needed.
Choosing a treatment that does not suit you wastes time and money, and can lead to distrust of the treatment itself.
If assessment shows Thermage is appropriate, the next question is how it is performed.
Thermage is not a matter of putting the device on the skin and pressing a button.
Whether the aim is preventive or corrective calls for an entirely different strategy.
Preventive: for someone in their late twenties or early thirties who wants to keep their current firmness. Here, rather than using very high energy, around 300 shots at a moderate level on a regular basis works well — like paying into a collagen account.
Corrective: where visible loss of firmness and fine wrinkles need improving. This needs at least 600 shots for the whole face, or around 900 including the neck. It also needs the highest energy level that stays short of a burn, to give the dermis strong thermal stimulation, if the improvement is to be meaningful.
Just as a sports car performs properly only with a skilled driver, results with Thermage vary enormously with the practitioner's experience.
Online reviews saying "it didn't work" or "I was burned" usually arise from treatment pushed beyond what the patient's skin could take, or from an inexperienced practitioner failing to control the energy precisely.
Do not be swayed by promotional reviews or cheap price events.
Your skin is never the same as the skin in someone else's review.
There are three things to check before treatment.
An accurate assessment: find a doctor who can tell clearly whether your concern is descended fat, loss of firmness in the dermis, or a lack of volume.
Genuine tips and genuine equipment: Thermage requires a single-use genuine tip as a matter of principle. Illegally reused tips are a direct route to side effects.
A sustainable plan: anti-ageing is a marathon, not a sprint. Rather than reacting to every short-term change, it matters to make a long-term plan that protects skin health five and ten years out.
We do not recommend treatments simply because they are fashionable.
We aim at sustainable anti-ageing — the best result by the safest route.
Thermage is without question an excellent device, but whether it is the right answer for you right now is something we would like to consider together.
