Search for anti-ageing treatments and you will find countless reviews of Thermage FLX.
Some call it the treatment of their life; others say they spent a great deal and saw no result. Why do outcomes differ so sharply with the same device?
Drawing on data from several thousand Thermage FLX treatments over the years, I want to set out how it works, how pain is managed, how side effects are prevented and who it suits.

Thermage FLX delivers strong radiofrequency energy to the dermis without damaging the surface.
Thermal stimulation contracts existing collagen loosened by age and induces new collagen, rebuilding the skin's foundation.
The most important thing to note is that Thermage does not produce an immediate change.
Change follows this timeline.
Immediately to 2 weeks: temporary tightening from thermal contraction is felt, but as swelling subsides it can feel as though the skin has returned to its previous state.
1 to 2 months: new collagen synthesis begins in earnest.
3 to 6 months (peak): the jawline is more defined, the texture smoother, pores tighter and firmness noticeably improved.
This is the general pattern and can differ with skin condition.
In short: Thermage FLX is a collagen-building treatment that asks for at least three months of patience.
Because Thermage is known to hurt, many ask for sedation — but for safety and the best result we recommend treatment while awake.
Accurate energy delivery: the resistance to RF energy differs with each person's skin thickness and water content. Communicating in real time and receiving feedback is what prevents burns and allows the maximum intensity suited to each person.
Pain management: instead of sedation we use a double anaesthetic system. A nerve block and a high-concentration anaesthetic cream applied under occlusion for at least 30 minutes to an hour reduces the pain. Airnox, one of the safe conscious methods, can also be used in addition.
The main reason people see no result is that the treatment did not suit their skin condition.
Suitability differs greatly with the pattern of ageing.

The average recommended interval is one year, and there is medical reasoning behind it.
The collagen life cycle: collagen produced after treatment is generated actively for about six months, then breaks down gradually over 12 to 18 months through natural ageing.
The stacking effect: repeating treatment at yearly intervals, before the collagen is entirely lost, lays new collagen on top of the existing layer. Treated consistently each year, this goes beyond preventing ageing — the skin's underlying resilience itself becomes younger.
An individual interval: where sagging is pronounced at the outset, a six-month interval may be better, so assessment with your doctor is essential.
Thermage FLX is a powerful, FDA-approved anti-ageing device, but it is not the right answer in every case.
Rather than being swayed by price or fashion, please check:
whether the clinic is transparent about using genuine tips,
whether the doctor accurately identifies the stage of ageing and the cause of the sagging,
whether they will firmly rule out treatments you do not need.
