For those who spend long hours at a monitor or on a phone, stiffness in the neck and shoulders has become very common.
Many also raise concerns about a raised shoulder line when wearing off-the-shoulder tops or dresses in summer.
Raised shoulders, a neck that looks short and chronic shoulder stiffness are often not simple accumulated fatigue but arise from excessive bulk and continuous contraction of the trapezius muscle.
Today I want to set out the medical basis of trapezius botulinum toxin, its aesthetic and functional effects, and the precautions for safe treatment.
Trapezius bulk arises as the muscle contracts excessively through stooped posture, forward head posture and continuous stress.
The trapezius is a large muscle running from the back of the neck to the shoulders and upper back, taking the leading role when we raise an arm or hold a posture.
Sustained poor posture — the head bent forward, the shoulders tensed — places excessive load on it.
As the muscle fibres enlarge, the neck looks shorter and the shoulders more angular, while internally the pressure within the muscle rises, causing chronic tightness and pain.
Injected botulinum toxin blocks the release of acetylcholine at the motor nerve, suppressing excessive contraction and reducing the muscle's bulk.
Once injected, the nerve signal is blocked and the over-tensed muscle enters a state of rest.
The muscle fibres, now used less, then undergo natural atrophy.
Aesthetic mechanism: as the volume of the enlarged upper trapezius decreases, the shoulder line softens and the collarbone becomes more defined.
Functional mechanism: as excessive tension and the accumulation of fatigue products within the muscle ease, chronic shoulder pain and neck stiffness are relieved.
The main target area, the dose and when the effect is felt differ according to the purpose of treatment.
Avoiding excessive dosing and using an appropriate amount prevents temporary weakness, and aftercare avoiding irritation helps the effect last.
Appropriate dosing (preventing weakness): the trapezius is a large muscle used for strength, so injecting an excessive dose at once can make it difficult to raise the arm or lift objects. An appropriate dose must therefore be given under careful assessment by an experienced doctor.
Understanding compensation by nearby muscles: between one and four weeks after treatment, as part of the trapezius function is suppressed, other small muscles in the back of the neck or the back take over the work. Temporary stiffness can be felt during this period, easing naturally as those muscles adapt.
Preventing spread and irritation: avoid strong massage or pressure on the shoulder area for about a month, to prevent the toxin spreading to nearby small muscles. Also avoid high-temperature environments such as saunas and steam rooms for about a week.
Correcting posture: correcting forward head posture and adding light stretching can greatly slow the rate at which the muscle enlarges again once the effect fades.
Q1. When does it take effect and how long does it last?
A. Shoulder pain and tightness begin to ease within one to two weeks, and the shoulder line becomes slimmer from two to four weeks. The effect varies with how much the muscle is used, but lasts an average of three to six months.
Q2. When should I have it before an important occasion?
A. The slimmest, smoothest shoulder line appears around three to four weeks after treatment, so we recommend treatment about a month before a wedding shoot or similar occasion.
Q3. How should repeat treatment be scheduled?
A. Regular care in consultation at intervals of four to six months, when the effect begins to fade, maintains both the slimmer line and the relief from pain.
